Primary Care Medical Homes

Primary Care medical homes are patient centered, providing consumers with access to a regular source of high quality, timely, comprehensive, coordinated health services which integrate health promotion, disease prevention, and chronic condition management and  link to specialty care when necessary.  Medical homes help to make for a more positive patient experience, improve the health outcomes, and reduce costs. 

Regional Work highlights some of the many efforts underway in our region to make it possible for low income populations to have access to high quality medical homes by strengthening the community-based systems of primary care across all jurisdictions.
