Health Information Technology is changing the face of healthcare. Whether as a tool of consumer empowerment or a means of connecting different healthcare providers to facilitate patient safety and continuity of care, it is critical that the benefits of health information technology reach all segments and sectors of the population.

Within the region efforts are underway to foster the use of electronic medical and health records, and the real time electronic exchange of health-related data and information among organizations to improve health service and quality. This section provides an overview of some of the work being done by the region’s primary care coalitions and safety net providers in two main areas: health information exchanges, and electronic medical and health records. The resource section at the end provides links to some articles and web sites about interesting and innovative work being done elsewhere.
