DC Primary Care Association
DCPCA is a nonprofit health reform organization whose mission is to facilitate the development and sustainability of an effective integrated health care system in the District of Columbia that guarantees access to primary health care and eliminates disparities in health outcomes. It is works to expand the capacity and quality of community-based primary care for low income residents through its Medical Homes initiative, addresses issues of health access through adolescent wellness, community health worker and emergency room diversion programs, and partners with a number of groups around health information technology projects to facilitate the provision of coordinated, high quality services.


Maryland

Primary Care Coalition of Montgomery County
The Primary Care Coalition of Montgomery County (PCC) is non-profit whose vision is that all county residents have the opportunity to live healthy lives, and that Montgomery County will be the healthiest community in the nation, serving as a model for providing access to high-quality and efficient care for all. PCC administers Montgomery Cares (for adults) and Care for Kids (for children), public/private partnerships tasked with providing primary and preventive healthcare to low-income, uninsured county residents at area independent nonprofit clinics.

Greater Baden Medical Services, Inc (Federally Qualified Health Center) Prince George’s County
Greater Baden Medical Services, Inc. is the only Federally Qualified Health Center (FQHC) operating in Prince George’s County. Is seven sites provide primary health services and facilitates health promotion/disease prevention activities effective, and for the individuals and communities served, regardless of ability to pay.

Healthcare Initiative Foundation
The Healthcare Initiative Foundation is a nonprofit, grant making trust governed by an independent, volunteer board of trustees. The Foundation's charter provides that grants be made solely to improve healthcare in Montgomery County, Maryland. We envision a Montgomery County whose residents are served by a high-quality, comprehensive, cost-effective and sustainable healthcare system. The Healthcare Initiative Foundation supports organizations that offer solutions to improve the quality and delivery of healthcare for residents of Montgomery County, Maryland.


Virginia

Northern Virginia Health Services Coalition
Northern Virginia Health Services Coalition is comprised of primary care providers committed to access to quality care for low income residents of Northern Virginia. We work together to develop coordinated, patient-centered, community-based, culturally appropriate health services. We aim to ensure 100% access and 0% disparities and systems in which providers and consumers show mutual respect, have open communications, and work together to achieve the best possible health outcomes.

Northern Virginia Health Foundation
The mission of the Northern Virginia Health Foundation is to improve the health of residents of Northern Virginia, with emphasis on supporting efforts to improve the health and health care of low-income, uninsured or underinsured persons, and on supporting the provision of health education, prevention of disease and wellness programs. The Foundation is also developing partnerships and strategic alliances with other local donors, nonprofit organizations, and key stakeholders from the private and governmental sectors.


Regional

Eugene and Agnes E. Meyer Foundation
The Meyer Foundation’s health portfolio funds organizations that advocate for an integrated health care system for uninsured and underserved people, help people access culturally/linguistically appropriate primary care, educate people about improving their health, or provide an integrated continuum of care—including primary care, mental health services, chronic disease management, and substance abuse treatment. Beyond grant-making, they work to strengthen nonprofit organizations and to address important issues that will improve the quality of life for people in the region.

Morris and Gwendolyn Cafritz Foundation
The Morris & Gwendolyn Cafritz Foundation is the largest independent grant-making foundation focused exclusively on the Washington Metropolitan area. We are committed to building a stronger community for residents of the region through support of programs in arts and humanities, community services, education, and healthcare. The Health portfolio focuses on Domestic Violence/Violence Against Women, Health Treatment Centers and Home Health Care, Mental Health, Public Health, Substance Abuse Prevention and Treatment, and HIV/AIDS Prevention and Education.

Consumer Health Foundation
The Consumer Health Foundation is a private health foundation that provides grants to local grassroots and community-based organizations in the metropolitan DC area. Our mission is to improve the health status of DC area communities -- particularly the most vulnerable members of these communities -- and to support activities that enable people to be more actively involved in their own health. Our work centers on reducing the racial, ethnic, and socioeconomic health disparities that exist in accessing healthcare and in health outcomes.

Kaiser Foundation Health Plan of the Mid-Atlantic States, Inc.
Improving the health of the communities it serves is Kaiser Permanente of the Mid- Atlantic States’ mission. Kaiser Permanente's Regional Community Grants program makes targeted grants to not for profit organizations, local governments and educational institutions in the region that align with specified focus areas.  Kaiser Permanente also sponsors health related programs, projects and community events. Current funding priorities include strengthening access to health care for vulnerable populations through support to safety net providers, public institutes and other  community based organizations,  and promoting Health Eating Active Living (HEAL) through partnerships with communities to improve access to healthy foods, supporting physical activity and enabling healthy lifestyle choices. Kaiser Permanente also helps make affordable healthcare available to over 5,000 uninsured and underserved adults and children through its Bridge Program and Medical Care for Children Partnerships.
